Nursing manager – Complex care

Our client has a fantastic opportunity within the areas of West Sussex and New Hampshire, for a Nurse who is looking for an opportunity in the community, assisting with care for adults and young people. Able to manage their diary and ensuring the service flows effectively. This opportunity has become available due to company progressions.

Key responsibilities:

– Report to the Head of Clinical Excellence.

– Ensuring a culture is built and maintained of clinical quality, patient safety, and clinical effectiveness, and monitoring its impact to ensure continuous improvement.

– Making and keeping relationships with Key External Health Partners/ Stakeholders (including CCGs, Specialist Commissioners, Local Authorities and Case Managers, local CQC and safeguarding).

– Ensure that services operate to the standard required by the regulators and the Codes of Practice of relevant Professional bodies.

– You will be a role model of the values, behaviours and standards expected of all employees.

– Supporting staff meet their regulatory bodies requirements and codes of conduct. Recognise training and development needs and action accordingly.

– Delivering a quality, patient-centred service, engendering a workplace culture that engages staff and service users to work collaboratively and in partnership at every level.

– Deliver services on time, within corporate and quality governance guidelines and within allocated resources.

– Provide nursing leadership to the team ensuring that all staff are actively engaged in delivering service user care in accordance with the agreed plan of care.

– Act as an expert practitioner within the team offering support, guidance, and training to team colleagues as appropriate.

– Ensuring that service users are empowered to personalise their care to their needs and to identify and achieve their aspirations.

– Complete clinical assessments, person centred care plans, associated risk assessments, monthly audits, care package reviews.

– Oversight and management of any MAR chart (where applicable) with evidence.

– Develop strong working relationships with MDT as and when it is required to ensure holistic care plan delivery.

– Liaise with the CCG and case managers as and when it is needed.

– Inducting new members of staff facilitating meet and greets with new service users and staff members.

– To complete competency signoffs, spot checks, clinical supervisions and probations Annual appraisals for staff working within packages.

– Ensure packages are adequately staffed Supporting other Nurse Managers.

– To do whatever it takes to ensure that POC are delivered safely and are fully staffed and if required, step in, to back fill unexpected absence.

– Manage and meet expected staff and client’s compliance in accordance with the CQC, other statutory bodies and policies & procedures.

– Ensure appropriate procedures are in place and adhered to for all clinical risk management.

This role will need excellent time management and diary upkeep as due to the nature of this work it is not a 9-5 Monday to Friday role. Occasional work outside these hours will be required to ensure the safety of staff and clients. Included in the role, you will be required to cover On-call duties as reasonably required by the business, this will be on an rota basis.
